No, because staging went fine.

When the booster flipped around and relit to boost back things went wrong, likely a result of the hot staging inducing a negative g-load on the booster causing fuel to slosh and then fluid hammer into the engines. Which is bad.

Fluid hammer due to re-lighting the engines also killed the Soviet N1 moon rocket.

More interested to learn what killed the upper stage. It was heading downrange just fine. Until it wasn't.
