So my 85 year old father has to pay an ambo (ambulance) bill in of $304 AUD.
if he were a pensioner, it would be free.

Gawd I'd hate to think what the US charges.

We are Americans living in France, and for our first five years here, we paid out of pocket and our private insurance reimbursed us. My husband had a trip in an ambulance to the hospital. We paid right then at the hospital. 89 euros, later reimbursed. Amazing. Vive la France !

I know that tourists are able to access our public health care system for free for emergencies or urgent cases. I do believe that if you require specialist care or treatment, then it isn't covered and tourists are required to get appropriate insurance for it.
