Hmm, I'm neurodivergent and often call people I don't know by their last names & didn't realize that would be offensive. Is this referring just to certain contexts? Like, as a patient to a doctor, or in general?

Your question really made me think! I think this was referring to online interactions, where men reply unsolicited to women, using their first name, usually to disagree. The same dynamic exists IRL, but it would be a hard to accidentally create in a conversation with someone you have respect for.
