The oldest human civilization from which we have written records is ancient Sumer. One thing we know from the copious amount of writing they left -- so much it isn't even all translated yet!-- is that they had a thriving and extremely varied transgender population.

Let's get into it:
There were people who were cis men at home but who worked as temple priestesses.
There were full-time transgender temple priestesses sacred to the Goddess Inanna.
There were trans male temple guards, also sacred to Inanna.

The oldest extant signed poem is by a temple priestess, Enheduanna, whose poem extols the many powers of Inanna, which include "turning a man into a woman and a woman into a man."
Sumer had nonbinary people and nonbinary mythical characters.
Sumer had gender-affirming surgery.
