One of the biggest things that has always bothered me about food safety is that recalls are largely voluntary. Companies can literally do a cost-benefit analysis and decide that settling potential lawsuits is cheaper than doing a recall and keep on doing what they're doing.

Regulatory agencies have a huge number of hurdles to go over to actually force a recall, and by then the harm is already done. It's not just food either, pretty much all consumer products. And they don't go to any effort to notify customers, they rely entirely on the media to carry the story.
