To those of us that learned to tell time before the advent of digital watches, it “a quarter to twelve.” For those that learned after, it’s “I can’t tell time without the numbers.” :) Or, if they managed to learn, it’s “eleven forty-five”

Depends on if I have something at 12:00. 11:45 is the time. A quarter to 12:00 is a mark of time within the context of a schedule.

Depends on how old the person you are telling the time to is. But I find them interchangeable.
