Say you need a screen reader (SR) to be online (not just for vision issues). When the SR gets to an image WITHOUT Alt text, it might say "image."

It ONLY reads TEXT, not an IMAGE of text.

Images with a quote mean nothing to an SR and the listener.

Cookie (my cat) has trying to be friends with Cocoa (dog) for a while. Cocoa has been rebuffing her because she doesn't understand Cookie’s head butts and attempts to snuggle. Last night I looked over and saw this.
